Our medical director designs a care plan with you — the visits, monitoring, and support you need to stay well at home.
Nurses and care coordinators visit regularly, check in between visits, and step in quickly if something changes.
The goal is simple: more good days at home, and fewer nights in a hospital bed.
Programs like Mount Sinai's Home Health Program in New York have shown what's possible when a physician-led team cares for seniors at home. Claire is building the same model, thoughtfully, for the East End.
